Regarding the procedure for requesting disclosure, etc.
Our company can receive requests from the persons concerned or their representative for notification of the purpose of use, disclosure, correction, addition or deletion of content, cessation of use, erasure and cessation of provision to third parties, and record of provision to third parties with regard to the personal information held by our company (hereinafter referred to as "Request for Disclosure, etc. "). 3. Identity verification
Our company will verify the identity of the person making the request for
disclosure by telephone. However, if telephone verification is not possible,
you may be asked to present a copy of your identity documents.
4. Request for disclosure by a representative
If you wish to authorize a representative to make a request for disclosure,
etc., please enclose the following documents in addition to the Request Form
for Disclosure of Personal Information.
(1) Documents to verify the identity of the representative (copy)
(2) A letter of attorney, which must be stamped by the principal, with the
certificate of seal registration attached. If the proxy is a legal representative
such as a person with parental authority, a document showing the relationship
with the person may be submitted in place of the letter of attorney.
5. Fees for the request for disclosure, etc. and notification of purpose
of use
A fee of 500 JPY (including tax) will be charged for each request for disclosure
of personal information and notification of purpose of use.
To pay the fee, please transfer 500 JPY (including tax) by bank transfer.
*We will provide you with the account information for the bank transfer upon
verification of your identity.
*Please bear the bank transfer fee by yourself.
Please note that we will not be able to disclose the information or notify
you of the purpose of use if the fee is insufficient or if we are unable
to confirm the transfer of funds.
6. Response to the request for disclosure, etc.
We will respond to your request by the method you have requested (e-mail,
fax, or mail).

If you are not satisfied with our company's response, you may file a complaint
